Medical problems

Obstetrical History:

Number of pregnancies
Mode(s) of delivery
Labor complications
History of miscarriage or elective abortion

Surgical history:

Abdominal surgery
Pelvic surgery

Social History:

Tobacco use
Drug use
History or risk of sexually transmitted diseases

Family History:

Diabetes
Hypertension
DES exposure
Thyroid disorders
Tuberculosis
Seizure disorders
Blood disorders
Twins or other multiples
Birth defects
Mental retardation

Thorough Physical Examination:

Pelvic exam
Pap smear
Palpation of uterus and ovaries to note any anomalies
Cultures for sexually transmitted diseases

Blood Tests:

Blood type and RH factor
Antibody screen
Complete blood count
Serologic testing for syphilis
Serologic testing for rubella
HIV (offered)

Urine Sample:

Urinalysis
Culture to rule out bacteria
Screening for abnormal levels of glucose, protein or ketones
